The Connection Center, A Referral Source to Services was established in September 2017, under the vision of Apostle Mark A. Henton, Pastor of the Monument of Faith Church.

Concerned with the statics of the city’s poverty level- at an all-time high, The Center responded to the ongoing issue of those in need on the southwest side and surrounding areas of Chicago. It helps bridge the various gaps by identifying the necessary resources, referrals, and solutions to address underlying issues. The Center also understands that connections to the right resources are a critical step for those in need. Helping to address, manage, guide, and support them through the process which can deliver a better quality of life. However, it’s almost impossible for those who are living in high crime areas and in poverty to sustain a quality of life without the necessary assistance. Intervention is needed.

The Connection Center has successfully provided referral sources that helped nearly 200 individuals/families identify resources to stop utilities shut-offs and evictions. These families were on the verge of losing their homes, mainly because of their change of income status, which resulted from the loss of jobs, theft health-related, and domestic violence issues.

The Center's mission focuses on helping provide families and individuals that are in crisis mode and need assistance, identify resources to meet their basic needs, and securing community-based support services through referral processes.

The Center has provided services in the Ashburn, Auburn Gresham, Chicago Lawn, Englewood, Gage Park, West Englewood, and the West lawn areas. It respects the dignity and worth of each person; practice compassion, respectfulness, mindful of individual differences, cultural norms, and ethnic diversity while providing service

The Connection Center became incorporated, 501 (c)(3) tax-exempt organization in June 2018. and is currently located and operated out of the Monument of Faith Church at 2750 W. Columbus Avenue on the south-west side of Chicago. The day to day operations is led by Jennifer Nelson Seals, current President.
